Medical forms for summer camp contain all sections that are to be filled out. For your convivence, Part C (for your scout's physician to fill out), is available as a single download for printing/emailing to your pediatrician.

Troop 121 attempts to camp monthly during the school year and is weather dependent. The senior patrol meets to plan ahead of where to camp and if any troop-related activities are to be conducted. Camping fees vary from site to site though are typically less than $50 per scout, lasting one to three days in length. Summer Camp is at the June Norcross Webster Reservation, with the Troop attending the first session (typically the last week of June). Fees and medical forms are collected by the Troop.
Parents are strongly encourage to complete the RSVPs (available on the calendar page once logged in) for each camping event when posted.
